Guidelines for the Kingsport Lions Club Eyeglass Assistance

 

1. An adult must have a Kingsport, TN. Mailing address and meet income guidelines to be provided service.

 

2. An adult who lives outside Kingsport, TN. but works in Kingsport, TN. and must have glasses to hold a job, and meets the income guidelines will be considered on a case by case basis.

 

3. Any student of the Kingsport, TN. school system less than 18 years of age and not covered by Tenn-Care whose family meets the income guideline will be provided service.

 

4. Students who attend Sullivan County School System who are under 18 years of age and are not covered by Tenn Care, whose family meets the income guidelines and lives west of state route 37 will be provided service. State route 37 runs from 11W at Central Heights thru Blountville to 11E at Thomas Bridge community.

 

5. The income guideline is $1000 per month per applicant for the primary applicant.  Additional family members living in the same household will be allowed $225 per month each.

 

6. The income guidelines will be reviewed annually in April prior to the new Lions Club fiscal year as part of the budgeting process.

 

7. Glasses will be provided to applicants only once every 24 months.  Requests for more frequent glasses will be looked at on a case by case basis considering the reason for the request.

 

8. Special or unusual requests for glasses will be reviewed by the 1st Vice President of the Kingsport Lions Club on a case-by-case basis.

 

STEP 1:  Make an appointment with the Sullivan County Health Department (423-224-1600) for a review of your qualifications.  They will send us a card if you qualify and we will contact you by letter with further instructions.
